Sciatica is pain that travels from the lower back or buttock down the back of the leg, sometimes into the foot. It can come with tingling, numbness or a heavy feeling in the leg. It is one of the most common reasons people visit our clinics in Shamsi Society and K.A.E.C.H.S, Karachi.
What causes sciatica?
Sciatica is a symptom, not a diagnosis. It happens when a nerve root in the lower back becomes irritated. Common causes include:
- A disc bulge or herniation pressing on a nerve root
- Narrowing of the spaces the nerves pass through (spinal stenosis), more common after age 50
- Long hours of sitting, heavy lifting or sudden twisting that flare an existing problem
Signs that need urgent medical care
Most sciatica settles with the right care. Go to a hospital emergency department straight away if you notice any of these:
- Loss of bladder or bowel control, or difficulty passing urine
- Numbness in the groin or between the legs
- Weakness in the leg or foot that is getting worse
- Symptoms in both legs at once
How physiotherapy helps
Most people with sciatica improve without surgery. At your first visit we examine your back, nerves, strength and reflexes, check for warning signs, and look at any reports you bring. Treatment usually includes:
- Specific movements and positions that ease leg pain, using approaches such as the McKenzie (MDT) method
- Guided exercise to rebuild strength and confidence in bending, sitting and walking
- Manual therapy and other techniques to ease pain where helpful
- Advice on daily habits, such as how to sit at work, sleep and lift
Should I rest in bed?
Usually not. Long bed rest tends to slow recovery. Staying gently active within your comfort, with the right guidance, is generally better.
